  • Insurance Card & Referral Letter (If Required)
  • Completed Patient Intake Forms
  • Any Reports, X-Rays, MRI, CT Scan & Other Relevant Information

What to Expect

  • Personal Consultation and Evaluation with Dr. Clare
  • On-Site X-Rays, typically taken in a standing position to assist in determining your diagnosis
  • Discussion of Non-Surgical & Surgical Treatment
  • On-Site Bracing or Casting if needed
  • Facilitation of Additional Studies (MRI, CT Scan, etc), Physical Therapy, Home Exercises, Orthotics, Other Specialty Consultations
  • Ample Opportunity to Ask Questions / Understand your Condition & Treatment Options

Cancellation of an Appointment

  • Call the office during business hours with at least 1 days’ notice so that we can offer your appointment slot to another patient.
